We’re a purpose-driven charity working to spark conversations that save lives - through storytelling, education, and lived experience.

 

Founded by heart transplant survivor Jayden Cummins, Forever Grateful was born from lived experience. Jayden knows what it means to be given 48 hours to live - and what it means to survive because someone, somewhere, said yes. What started as a deeply personal journey has grown into a national mission: to start life-saving conversations before it’s too late.

 

We share stories that matter - stories of survival, loss, resilience, and second chances. Because while the facts about organ donation are important, it’s often a single, honest story that opens the door to a conversation families might otherwise avoid.

What We Do

We raise awareness and inspire action through:

• In-Person Talks in schools, workplaces, and communities

• Short Films and Educational Videos for classrooms and national campaigns

• Supporter-Led Initiatives like Survivor Shirts, which help fund life-saving cardiac and transplant research

 

At the heart of it all is our belief that emotion creates connection - and connection creates change. We use real stories to drive real change. Whether it’s a talk in a school, a short film on social media, or a shirt worn with pride, everything we do is designed to spark conversation, shift perception, and support the science that saves lives.

 

Why It Matters

Nearly half of all Australian families decline organ donation when asked - not because they’re against it, but because they simply never had the conversation. Families are left to make impossible decisions at a time of immense grief without knowing what their loved one wanted. Forever Grateful exists to change that - because with the right story, told at the right time, everything can be different.

 

Because one story can change a mind. And one conversation can save a life.
